Job Description
Community Health Network (CHN) has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer flexible work options and outside of the box opportunities that focus on your needs and interests.
We are currently assisting one of top healthcare system partners with a need for the following in the Care Progression Unit.
Great opportunity for an experienced Home Health VNA RN who may be interested in working on the inpatient side of Case Management. Hospital based, no travel in the field required. Specific training needed will be provided for the right candidate.
Position: RN - Registered Nurse - Case Management
Location: Weymouth, MA
Shift: Days
Hours: Monday-Friday, 40 hours/week, every other weekend rotation
- Minimum 2 years Home Care VNA experience
- Must have previous discharge planning experience and home equipment set up
- Inpatient hospital experience not required!
