Responsibilities:
· Conduct comprehensive patient assessments through interviews, examinations, and reviewing clinical data
· Develop individualized care plans aligned with patient needs
· Collaborate with patients, families, and interdisciplinary team members
· Provide professional, skilled nursing services in patient homes
· Ensure proper documentation adhering to guidelines
· Respond to emergency situations following protocols
Requirements:
· Graduate of an accredited Nursing program
· Active Compact or New Hampshire RN license
· CPR certification required
· EMR experience preferred
· Excellent communication and clinical skills
· 1 year of home care nursing experience strongly preferred
