Currently we are assisting one of our top home health agency partners with a need for an experienced Home Health RN Case Manager to see patients in Assisted Living Facilities in locations across Guilford and Forsyth counties.
Monday-Friday; 32 hours guaranteed
On-call; 1 week/month (Friday-Thursday)

The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
Requires minimum 2 years of recent Home Health RN Case Management experience doing OASIS visits with a Medicare certified agency.
