Community Health Network (CHN) is assisting a top home care agency with a need for an experienced Hospice RN for an after hours role. This would be a home based on-call position. This is a full-time position, 40 hours/week.
Shifts Available: Evening shift (3:00p.m.-11:00p.m.) or Night shift (11:00p.m.-7:00a.m.).
Work Week: Monday-Friday
Responsibilities: Phone triage, field visits and assessments as needed.
(No case management required)
Requirements: Active MA and RI RN license
2 or more years current Hospice RN experience
Ability to travel to patients homes and/or facilities after hours during the evening or night.
