Currently assisting a well established hospice agency with a need for a Hospice RN Case Manager to see patients primarily in facilities located in the Mansfield, Franklin, Walpole, Sharon, MA area.
Full-time, Monday-Friday, 40 hours/week
No weekends or on-call required!
Great rate plus mileage reimbursement.
Requires minimum 1 year of Hospice RN experience with a Medicare certified agency.
Primary nurse provides quality nursing care to identified Hospice patients. The scope of the primary Hospice RN includes direct patient care, coordination of patient and patient advocacy. Patient includes the individual, family/significant other and community. Must be comfortable with issues relative to end-of life care, death and dying.
