Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health RN Case Manager.
Coverage needs primarily in and around the St. Augustine/Jacksonville FL areas. (St. John/Duval counties)
Full-time, Monday-Friday, 40 hours/week
On-call rotation may be required
Competitive payrates up to $52.00/hour with IRS mileage reimbursement
Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
