Currently we are assisting one of our top home health agency partners with a need for an experienced Home Health RN Case Manager.
Primary coverage territory needs are between Milford, MA and Marlborough, MA areas.
Monday-Friday
May cover on-call/weekends as need
Competitive pay rates plus mileage reimbursement

The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
Requires minimum 2 years of recent Home Health RN Case Management experience doing OASIS visits with a Medicare certified agency.
