Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health RN Case Manager.
Coverage territory needs include Chapel Hill/Carrboro/Morrisville/Pittsboro/Cary, NC.
Monday-Friday-Days
No holidays!
On call rotation every 8 weeks (one full week, M-S)
Competitive payrates plus mileage reimbursement
The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
Requires minimum 2 years of recent Home Health RN Case Management experience doing OASIS visits with a Medicare certified agency.
