Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health RN Case Manager.
Coverage territory needs for the Bridgewater/Raynham MAarea.
Monday-Friday-Days, 40 hours/week
On-call may be required if needed
Competitive payrates with mileage reimbursement
Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
