Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health RN Case Manager.
Coverage territory needs include Fulton County, East Atlanta, West Atlanta, Midtown.
Monday-Friday
Full-time, contract
Occasional on-call if needed
Competitive payrates plus mileage reimbursement
Local hourly rates or up to $1,840 weekly gross Travel packages
The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
Requires minimum of 1 year of recent Home Health RN Case Management experience doing OASIS visits with a Medicare certified agency.
