Currently assisting one of our top home care agency partners with a need for an experienced Home Care Registered Nurse (RN) Case Manager to see patients in Wenham/Danvers/Topsfield, MA and surrounding areas.
- Annual salary range $110,000 - $120,000
- Mileage reimbursement

The Home Care Registered Nurse (RN) Case Manager is responsible for effectively managing and caring for a caseload of patients in the community, from admission to discharge.
- Requires 1 year of recent Home Care Registered Nurse (RN) Case Management experience doing OASIS visits with a Medicare certified agency.
