Currently assisting one of our top home care agency partners with a need for an experienced Home Care Registered Nurse (RN) Case Manager for coverage needs in the Hyde Park, Jamaica Plain, West Roxbury, Roslindale, MA areas.
- Annual salary range $100,000 to $120,000
- No weekends, on-call, or holidays required!

The Home Care Registered Nurse (RN) Case Manager is responsible for effectively managing and caring for a caseload of patients in the community, from admission to discharge.
- Requires 1 year of recent Home Care Registered Nurse (RN) Case Management experience doing OASIS visits with a Medicare certified agency.
