Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care RN Case Manager.
Coverage needs primarily in and around the Weymouth, Bridgewater, Canton, Milton, Quincy, MA areas.
Monday-Friday, 40 hours/week
Every 6th weekend rotation

The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
