Job Description
Community Health Network (CHN) has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer flexible work options and outside of the box opportunities that focus on your needs and interests.
We are currently seeking an experienced Home Care Physical Therapist to care for patients in the Jamaica Plain/Dorchester MA area.
***Requires 1 year of recent Home Care Physical Therapy experience doing visits in Home Health with a Medicare certified agency. **
Full-time, Monday-Friday, 40 hours/week
Occasional weekend rotation may be required
No on-call
