Community Health Network is seeking an experienced RN for a full-time Director of Nursing role overseeing a new Home Care service line. As the Director of Nursing for Home Care, you will play a pivotal role in ensuring our clients receive top-notch care while creating a dynamic atmosphere that encourages professional growth, accountability, and autonomy.
Key Responsibilities:
Quality Patient Care: Lead the charge in delivering exceptional care by developing and nurturing a professional environment that prioritizes excellence and patient-centered services. In this highly visible role, you will create and build teams, develop relationships, and form community partnerships that align with the strategic growth plans in a start-up atmosphere.
Regulatory Compliance: Prepare the agency for state licensure surveys, ensuring all clinical staff meet and exceed state and federal regulations. Your expertise will keep us ahead of compliance standards, maintaining our reputation for high-quality care.
Performance Improvement & Education: Champion performance improvement initiatives and provide essential education to staff, ensuring adherence to all policies, procedures, standards, and relevant laws and regulations.
Best fit candidates for this role would have a demonstrated track record of the following:
· Exceptional communication skills and with a focus on providing the best customer and candidate experience at all times.
· Experience with case management and the oversight of care plans.
· Experience in the field doing Supervisory visits and/or reassessments with caregiver present.
· Exceptional time management skills with the ability to delegate as needed.
· Knowledge of Home Health Federal and State regulations as well as utilizing resources and active participation staying informed as to any updates or changes.
· Knowledge of private pay, insurance, government payment and coverage options for in home care.
· Comfort level and experience working with community partners and building relationships.
· Ability to problem solve and create learning opportunities for both internal and external employees.
· Ability to be a hands-on Manager when needed as well as build teams.
· Understanding of operational finances and budgets.
· Experience with EMR training and usage.
Requirements:
At least 5 years of nursing experience, with and 3 years in either Home Care, Geriatrics, Care Management or combination.
Knowledge of the NH State and Federal rules and regulations for Home Care
Bachelor of Science Degree preferred
Experience performing patient assessments
Excellent communication and interpersonal skills
Unencumbered New Hampshire RN license
