Position Overview
A growing home care agency in the Needham, MA area is seeking an experienced Clinical Nurse Manager (RN) to lead and support clinical operations. This role is ideal for a hands-on leader who thrives in a fast-paced environment and is passionate about delivering high-quality, patient-centered care in the home health setting.
The Clinical Nurse Manager will oversee clinical staff, ensure compliance with Medicare-certified home health regulations, and drive excellence in patient outcomes, staff performance, and operational efficiency.

Key Responsibilities
· Provide day-to-day clinical leadership and supervision of field nurses and clinical staff
· Ensure compliance with Medicare Conditions of Participation (CoPs) and all state/federal regulations
· Oversee clinical documentation, OASIS accuracy, and care plan development
· Conduct case conferences, chart audits, and quality assurance reviews
· Collaborate with intake, scheduling, and administrative teams to ensure seamless patient care delivery
· Support staff onboarding, training, and ongoing performance management
· Serve as a clinical resource and mentor to nursing staff in the field
· Monitor and improve patient outcomes, satisfaction, and quality metrics
· Participate in survey readiness and accreditation processes
· Assist with growth initiatives, including program development and service expansion
