RN Case Manager-Primary Care-Part-time
RN manages all aspects of patient care through multiple care delivery systems, acting as the central point of contact for patients, supporting them through transitions of care. Receives case management reports and primary care physician (PCP) referrals to target very high and high-risk patients to ensure quality care and patient safety. Through initial contact and assessment, establishes active partnerships with patients and clinical partnerships with PCPs, specialists, internal medicine (IM) leadership and staff, and internal and external resources. Reports to the Associate Manager of Case Management, with a functional reporting relationship to the site IM Nurse Leader.
Office based part-time position (3 days/week). Some flexibility on the days and hours worked.
Minimum 3 years’ nursing experience in case management/continuing care in an inpatient or community setting strongly preferred.
Experience with an electronic medical records system, EPIC experienced preferred. Must exhibit strong critical thinking, problem solving, interpersonal and good clinical assessment skills. Highly motivated and self-directed. Ability to interpret clinical information, assess the implication of treatment and develop and implement a plan of care. Demonstrated ability to prioritize, multitask, and work in a rapidly changing environment with multiple demands. Ability to work effectively in a team. Strong computer skills.

With her over 24 years of experience, 12 of which have been with Community Health Network, Katie is an expert in customer service and the “go to” person within the office.
