Assessment RN-Managed Care-Full Training Provided
Great opportunity for seasoned Home Care RN with strong admission/assessment skills looking for a change. This is a full-time temporary position with a top health care plan to conduct daily newly enrolled member assessments in patients homes in either north or south Worcester county. The Assessment Nurse utilizes advanced clinical judgement and critical thinking skills to facilitate appropriate physical and behavioral health care and social services for members through assessment, member centered care planning and integrated team collaboration to promote effective utilization of available resources, optimal member functioning, and cost-effective outcomes.
Full training provided for qualified applicants. Great pay rates and mileage reimbursement.
- Minimum 5 years Home Care RN Case Manager experience
- Strong assessment skills and knowledge of long-term care, behavioral health and chronic illness.
- Active MA RN License

Michelle Murray has been President and CEO of Community Health Network since she purchased the company from Kathleen McDonough in January 2018. Michelle brings a background of acute care hospital nursing experience from Brigham and Women’s Hospital and Yale New Haven Hospital combined with over 20 years in healthcare staffing and recruitment. Michelle looks to take what is already a quality driven organization and introduce new opportunities and ways to address the needs of both her staff and clients.
Michelle is a member of the Home Care Alliance of Massachusetts and the Massachusetts Home Health Executive Leadership Circle. She is certified in Interview Management and is a NAPS Certified Personnel Consultant.
Michelle received her Bachelor of Science in Nursing from Boston College. She lives in Bolton, MA with her husband and three children.
